Registration Instructions

0. On the registration form, indicate who will be attending and mark the events you and everyone in your party who will attend. Print names as you wish them to appear on the name tags.
0. Designate each person attending with an M(ember), A(ssociate member), P(artner), C(hild) if the person is under 12.
0. If you desire confirmation, please check the “Send Confirmation” box on the form and provide your name, phone, along with your email address.
0. Calculate the total cost for all events using the form. Please note that because we do not charge a registration fee and because caterers (unlike the Monticello Association) do not charge different prices for children than for adults, part of the costs of room rental, audio equipment rental, setup charges, and child subsidies have been factored into these prices.
0. Send form and check for the total amount, made payable to The Monticello Association
